How the appointment work?

Our appointments are on a first come first serve basis. We have a scheduled time for each doctor and patients

Do I have to have referral from my primary doctor?

If your insurance requires you to have a referral and we do not have one when you arrive, we will not be able to see you. We always recommend patients to be responsible for asking for and bringing their own referral themselves, just in case we have not received it from their primary doctor. We hate to turn patients away, please contact your insurance company and ask if you need a referral in case you are not sure.

What Info I should bring on my appointment day?

Please bing your ID, Insurance card & Info.

Your primary doctor's name, address and phone number.

Bring your medical history including dates you have been hospitalized or have had any surgery.

How do I request my medical records?

How do I refill my prescription

Have your pharmacy information ready when you call our office. Our staff will talk to our doctors first in order to fulfill your request. If you have been missing consultations, depending on the medication, it is possible the doctor may need to physically see you before accepting your request. Our staff will contact you and update you or you may call our office for update.
